If you are unsatisfied with services we provided, we want to know about it so that we can address it and try to make things right. Often problems can be resolved directly with your provider and we encourage you to speak with them first before filing a grievance. 

 

The information you submit on this form will be reviewed by both of the owners of Thrive Counseling and Consultation, LLC, Demetrius Williams and Julie Williams. You will receive a response within 7 calendar days of receipt of your inquiry. We appreciate that this is a sensitive subject, but it would be helpful for us to have as many of the details as you can provide. If we have additional questions, we will reach out to you by phone or email. We may choose to consult with other qualified professionals without disclosing any HIPAA-protected patient information in order to respond in the best manner possible.
